Job Description
PACT is seeking a dedicated and experienced Clinical Liaison Manager to join our team.
This pivotal role focuses on building strong connections between whānau and external clinical teams, ensuring safe, high-quality supports and smooth transitions for our community clients across the Lower North Island and Upper South.
About the Role
As a Clinical Liaison Manager, you will:
• Work closely with whānau and external clinical teams to strengthen collaboration and care pathways.
• Partner with community agencies, including Oranga Tamariki, to support children, young people, and families.
• Apply clinical expertise within mental health or disability support services to deliver tailored, effective outcomes.
• Manage transitions safely and effectively, ensuring continuity, clarity, and strong communication for all parties.
• Lead transition planning and support best practice approaches that uphold safety, client autonomy, and whānau engagement.
• Work with NASCs and other referring agencies to support informed decision-making and coordinated service responses.
• Use experience to guide best practice, contribute to service improvement initiatives, and provide insight into system-level quality enhancements.
• Apply this expertise to individual case reviews where needed, supporting robust, clinically-grounded decision making.
• Oversee quality control to maintain high standards of care and support.
• Champion improvements in primary health access and responsiveness for community clients.
• Educate and mentor others, sharing knowledge and fostering professional growth.
• Bring a “can do”, solutions-focused approach to every challenge.
Desired Skills and Experience
About You
We’re looking for someone with:
• 6+ years of clinical experience in mental health, disability support, or related fields.
• Experience working across specialty mental health or disability services is highly beneficial.
• Strong communication skills with the ability to engage effectively with whānau, clients, NASCs, referrers, and professionals.
• Experience working with Oranga Tamariki is beneficial but not essential.
• A passion for education, development, and knowledge-sharing.
• Experience relevant to guiding best practice, reviewing service processes, and contributing to quality improvement.
• The ability to apply clinical expertise in complex case reviews to support safe and sound decision-making.
• A commitment to safe, high-quality supports and seamless transitions.
• The ability to thrive in a non-management role focused on clinical excellence and influence.
